I think that the definition of Healthcare IT (Or Healthcare IS, HIS, HCIS, etc.) is inclusive of all provider types, so wether you're a doc in a box, Dr. 90210, the nursing home down the street, a large Academic medical center, the community hospital, critical access, the St. Paul Island (AK) clinic, or a home health agency, you'll want to come here for your answer. We (non-for profit hosp) are in the process of implementing a product that allows us to share information with practicing physicians in the area. Primarily by HL7 traffic. If they have an EMR we work with the practice vendor to get the messages to populate directly in to the pts record at the EMR. Check out Medicity/NOVO GRID – Anonymous Jun 25 '10 at 18:00
It is convenient and easy to implement point to point interfaces between healthcare providers ( hospitals, physician office and others), but is it the right approach ? NO. Federated architecture using IHE XDS or webservices using SOA architecture is the right approach to share patient records between healthcare providers. There are large parts of hl7 standards documentation that must be obtained by being a member, which costs about $600/year. If you google various standards though, you can find many of them, especially the older, commonly deployed ones, or at least, specific implementations of them. Some, like the EHR Functional Model, are publicly available. – Peter Harrison Jun 25 '10 at 13:30
check out Corepoint Health (I beleive they used to be NEOtool). If you are willing to pay for training they offer a few different types HL7 certification courses and resources. corepointhealth.com – Anonymous Jun 25 '10 at 17:57

If you are just looking to get started with interoperability in Healthcae and introduction to HL7 version 2.x and 3, check this book by Tim Benson shopcreator.com/mall/infopageviewer.cfm/abiescouk/… You can download parts of book especially chapters on HL7 v2.x and 3.0 Also this gives you context on where and how HL7 standard can be used. For each action, look for trigger events and outputs. Dietary orders can go to the department via phone, pneumatic tube system, printer, electronically, or a volunteer can show up and ask. I always ask a person, "How do you know to do that?" "How else can they tell you?" "What if they..." Don't over-analyze. Sometimes the simple solution is the cheapest and easiest way to get the job done. Above all: Listen to and believe your end-users! – Anonymous Jun 25 '10 at 18:15

How do you document your interfaces? To what level of detail? I.e. just IP/msg type routing or what fields are changed in translation/scripting?

If you are talking HL7 interfaces, we do both. We have ROUTING tables that assign message types/events to Q's which in turn subscribe to translators. There are no translations done in the ROUTER, it just routes to Q's. All translation is done with in the translators for each system we are sending to. We comment our code changes within each translator, especially when we are doing things out of the norm. All of this depends on the flexibility of the Engine you are using and philosophy surrounding the build of interfaces and transferance of your data. – Anonymous Jun 25 '10 at 18:12

Become a member of HIMSS, attend HIMSS conference, network in the industry 3. Join a Healthcare IT Vendor - thats how I started in year 2000 4. By offering a very reliable support model and keeping the total cost of ownership low compared to the licensed software options. Just like JBOSS, MYSQL and Red Hat did it on middleware, data and OS stack.
